Optimal neoadjuvant treatment strategies for patients with locally advanced rectal cancer (cT3-4NxM0) remain unclear, particularly in regard to chemotherapy as opposed to radiotherapy. Recent studies have suggested a possible benefit of combined chemoradiotherapy. The aim of this study was to compare the oncological outcomes of chemoradiotherapy to those of radiation alone in patients with high-risk rectal cancer. From a database of 1327 patients treated with preoperative radiotherapy for rectal cancer at a single centre between 2000 and 2009, a subgroup of those patients with locally advanced disease (cT3-4NxM0) were identified. The study group comprised 32 patients treated with chemoradiotherapy and the control group comprised 20 patients treated with radiotherapy alone. Three- and 5-year actuarial rates of local recurrence and disease-free survival (DFS) were compared between the two groups. The radiation dose administered in the chemoradiotherapy group was 44 Gy. Of the 32 patients in the chemoradiotherapy group, 10 patients received adjuvant chemotherapy. Adjuvant chemotherapy did not significantly affect 3- and 5-year DFS rates (95.2 vs. 90.0% and 85.0 vs. 73.2%, respectively). The only patient with local recurrence had received radiotherapy alone. For patients with stage III rectal cancer, 3- and 5-year DFS rates were 83.3 and 65.9% for patients receiving chemoradiotherapy and 64.5 and 40.3% for those receiving radiotherapy alone.
